Situated along the Trans-Canada Highway between the neighbouring city centres of Winnipeg and Thunder Bay, the District of Kenora is comprised of several communities in Northwestern Ontario, offering an experience in nature like no other. With its main administration office in Dryden and other office locations and ambulance bases across the District, the Kenora District Services Board (KDSB) is the integrated service delivery agency dedicated to improving everyday lives through early learning and care, housing, paramedic, and social services.
Through our continued growth, we are currently seeking to hire a Regular, Full-Time Integrated Community Case Manager located in Sioux Lookout, Ontario, to join our Integrated Social Services (ISS) team. Reporting to the Integrated Social Services (ISS) Supervisor, the Integrated Community Case Manager provides coordinated and ongoing case management that will support improved outcomes through connections to additional services and life skills training, including, but not limited to, financial support, employment readiness, housing stabilization, health support, and safety planning for the residents of the Kenora district.
KEY RESPONSIBILITIES INCLUDE:
- Plan, coordinate and promote integrated training forums in partnership with other community organizations.
- Identify, organize and facilitate training sessions as determined by individual and community need in order to support positive outcomes.
- Work in partnership with the internal and external stakeholders to provide outreach and meet individual and family needs.
- Assess individual needs, provide information to support individual improvement plans, and evaluate the plans’ ability to improve outcomes.
- Collect and provide statistical data to support program reporting and efficient, effective program design.
- Support individuals in collaboration with internal and external stakeholders to establish goals and achieve improved outcomes.
- Investigate alternative methods or partnerships for service opportunities and pathway development to optimize efficiency and improve individual and family outcomes. Refer individuals and families to services and support as required.
- Work with all KDSB programs using an integrated service delivery approach.
- Promote excellence in case management by appropriately documenting the services, supports, goals and outcomes with individuals and families.
- Maintain current knowledge and awareness of relevant legislation, policies, professional human service techniques and practices, community services, supports, and opportunities that benefit individuals and families.
SUCCESSFUL CANDIDATES WILL HAVE:
- Demonstrated excellence in customer service, communication, and interpersonal skills in dealing with individuals, co-workers, management, the public, employers, and other community agencies.
- Ability to assess risk and potential risks before meeting with individuals and families.
- High level of ability to use computers and other information technology, including specific case management software and administrative systems, including databases, word processing, spreadsheets, data input, and retrieval.
- Strong working knowledge of applicable legislation and acts that fall within the scope of the ISS department, along with knowledge of community programs to assist individuals with their needs.
- High level of ability to manage competing priorities and the ability to be flexible and multitask depending on the changing needs of individuals and families.
- Demonstrated knowledge of a large variety of community programs in order to assist individuals and families with their needs.
- Demonstrated ability to relate empathetically and effectively in order to motivate individuals and families
- Demonstrated skills in conflict resolution, leadership, and marketing.
QUALIFICATIONS WILL INCLUDE:
- Successful completion of a post-secondary education program in Social Services or similar and three years’ experience in social services. Alternatively, a combination of education and/or experience is required.
- A satisfactory Police Vulnerable Sector Screening.
- A valid Ontario Class G (or equivalent) driver’s license.
- Legally eligible to work in Canada.
The starting hourly wage for this position is $34.02 in accordance with the Collective Agreement.
